Meet Our Team

We provide the highest quality end of life care to patients and families through the use of the interdisciplinary team.

Your Hospice Team Includes

Medical Director – works directly with your Attending Physician to coordinate your plan of care, provides effective pain and symptom management and collaborates with the interdisciplinary team on your individualized plan of care to mare sure your physical, psychological, emotional and spiritual need are being met.

Skilled Nurses– regularly assess your needs, manage symptoms and report changes to the Attending Physician and/or Medical Director. Skilled Nurses will also teach, supervise and counsel you and your family members on providing care.

Social Worker – provides counseling, emotional, bereavement support and will assist you with financial concerns and community resources.

Spiritual Counselors – provided spiritual, bereavement and emotional support to you and your family members when needed. Assist with funeral or memorial arrangements and work with community clergy.

Hospice Aides – provide personal care and light housekeeping as well as emotional support and compassionate care to you and your family members.

Volunteers – provided support and companionship to you and your family members. Trained Volunteers can provide services as running errands, grocery shopping or providing respite services to caregivers and family members.

Additional Hospice Services Provided:

Bereavement Services are available to you, family members and caregivers. These services can include grief counseling and community support groups.

Therapy Services: Physical, Occupational and Speech Language Pathology services are available to help you in maintaining activities of daily living as well as provide training in how to use adaptive equipment, instructing caregivers and families on good body mechanics for turning and lifting patients and home safety.

Medical Equipment and Supplies, including Medications used for relief of pain and symptom control related to your terminal illness.
